Heartland Hospice seeks a Nurse Practitioner to deliver patient-centered care in both hospice and palliative settings. You will perform management visits, hospice Face-to-Face encounters, and collaborate with physicians to ensure compliant, high-quality care.
You will work with an interdisciplinary team to develop care plans, manage complex symptoms, and support patients and families through end-of-life transitions. A strong background in hospice/palliative care and MSN is preferred.
