Position Overview:

Key Responsibilities:Conduct comprehensive assessments, deliver skilled interventions such as wound care, infusions, and medication management, educate patients and caregivers, monitor progress, communicate with care teams, coordinate services, and maintain accurate documentation to ensure high-quality outcomes.
Qualifications & Skills:Active RN license, at least one year of clinical experience, patient-focused, organized, with a valid driver’s license, reliable transportation, and CPR certification or ability to obtain it.
