Agrace, a nationally recognized nonprofit leader in hospice and palliative care, seeks an RN to provide patient- and family-centered care in new markets, evaluate hospice eligibility, and admit patients to Agrace’s homecare program.
You will collaborate with the IDT, develop and update plans of care, coordinate with physicians, manage symptom control, and educate families, with on-call and rotating shifts supporting compassionate end-of-life care.
