Case Worker- CLTS Support & Service Coordinator

Grounded first in Eau Claire County’s mission and values, the Children’s Long-Term Support (CLTS) Case Worker partners with children with disabilities and their families to assess needs, develop individualized service plans, and coordinate supports that promote safety, independence, and community inclusion. This role carries out responsibilities in accordance with Wisconsin CLTS Waiver Program requirements, while prioritizing the needs of children and families.

The CLTS Case Worker monitors service delivery and collaborates with providers, schools, and community partners to achieve positive outcomes.

• Supports children with disabilities and their families by promoting safety, stability, and community inclusion; maintains a strong emphasis on keeping children in their homes and communities and preventing out-of-home placement whenever possible.
• Engages families through regular face-to-face and other contacts, including visits in the family home; meets families where they are at by building strong, trustingrelationshipsand utilizingmotivational interviewing techniques to support engagement, empowerment, and lasting change.
• Coordinates and facilitates team meetings, including family-centered planning and collaboration with providers, schools, and natural supports to ensure services are aligned, effective, and responsive to family needs.
• Coordinates services in a collaborative manner that maximizes effective use of resources, avoids duplication, and supports outcomes aligned with individualized, family-centered goals.
• Completes thorough, strengths-based assessments to identify the full range of child and family needs across medical, behavioral, social, educational, and daily living domains; partners with families to develop goal-oriented, time-limited service plans.
• Maintains knowledge of CLTS waiver services and community resources; educates children and families on available supports, rights, and responsibilities; empowers families to access services independently while coordinating and monitoring service delivery.
• Demonstrates effective case management practices aligned with CLTS Waiver Program requirements and best practices; conducts regular reviews, updates service plans, monitors service provision, and supports successful transitions or case closure when goals are achieved.
• Adheres to all documentation, confidentiality, and reporting requirements; completes accurate and timely records in accordance with CLTS and Medicaid waiver standards; prepares required assessments, reports, and correspondence.
• Establishes and maintains effective working relationships with internal and external partners; serves as an advocate for children and families; participates in team meetings, supervision, and community collaboration efforts.
• Maintains professional growth through ongoing training and development related to supporting children with disabilities and family-centered care.
• Conducts proactive family outreach to engage children and families in services, including consistent follow-up, flexible communication methods, and persistent efforts to reduce barriers to participation and connection.
• Performs other duties as assigned.

• Bachelor’s degree in social work or a related field.
• Appointment will be conditional upon successful completion of criminal and caregiver background checks.
• Must have valid driver’s license, an acceptable driving record, and access to private transportation for work-related duties.
• Mustpossessand maintain personal automobile insurance in the amount of $100,000/$300,000.

• Six (6) months of experience in family, children, or adult case management services, or successful completion of an internship serving children, youth, or families.
