Humanitas DMH zoekt een coördinator voor de Homerunhuis, gericht op jongeren tussen 16 en 25 jaar met LVB en multiproblematiek. Je bewaakt zorginhoud, regelt intakegesprekken en stuurt trajectplannen aan in samenwerking met collega’s, de gedragsdeskundige en de manager.
Je onderhoudt contacten met externe partijen, implementeert besluiten en bewaakt kwaliteit van zorg, met aandacht voor relatie en samenwerking. Ruimte voor professionele ontwikkeling en groei binnen het team wordt geboden.
