Compensation: $120,000 to $160,000+ Total Compensation (Base Salary + Productivity Incentive Program) +

- Deliver on‑site primary care and rounding services throughout senior living communities.
- Manage pressure injuries, diabetic wounds, post‑surgical wounds, and complex chronic conditions.
- Coordinate care with physicians, nursing teams, therapists, caregivers, and facility leadership.
- Support preventative care initiatives, chronic disease management, and continuity of care.
- Help improve healing outcomes while reducing avoidable hospitalizations and complications.
- Active Minnesota Certified Nurse Practitioner (CNP) license.
- Experience in wound care, geriatrics, primary care, post‑acute care, or related settings preferred.
- Valid driver's license and ability to travel throughout the Twin Cities Metro Area.
- Strong clinical judgment and confidence managing medically complex patient populations.
