About the Role
The Nurse Practitioner (NP) operates on our clinical operations team, providing direct patient care across our medical centers, nursing homes, skilled nursing facilities (SNF), and home settings. The role supports geriatric and general acute care, case management, and collaborative practice in partnership with physicians and the interprofessional team.
Responsibilities- Conduct geriatric assessment, medical history, and physical examination
- Diagnose and treat acute and non‑acute conditions
- Develop nursing plan of care, health education, and patient counseling
- Order and interpret laboratory/diagnostic studies
- Provide physician referrals and case‑management referrals
- Write admission, transfer, and discharge orders
- Participate in clinical rounds, conferences, and risk/quality management programs
- Document care through progress notes and summaries according to Chen Med standards
- Perform invasive procedures upon documented competency
- Initiate life‑saving procedures and recognize situations requiring physician intervention
- Serve as a patient advocate and facilitate family conferences on goals of care, palliative, and hospice decision‑making
- Collaborate with multidisciplinary team members to transition patients to lower levels of care promptly
- Implement new practice patterns to improve systems of health care delivery
- Analyze and evaluate current health‑care delivery systems
- Participate in professional development activities
- Coordinate SNF discharge arrangements and paperwork
- Prescribe medication as permitted by state scope of practice
- Complete additional duties as assigned or modified by the manager

- Proficiency in patient chart review, medication reconciliation, and follow‑up

- Ability to travel locally 50–75% of the time
Salary: $111,140 – $158,771 annually. Final compensation will depend on experience, education, location, and other factors. Eligibility for bonuses or commissions may apply.
