Role Description
Oak Street Health is an innovative, community‑based healthcare company providing high‑quality primary care to adults on Medicare. We are currently seeking a Registered Nurse (RN) to join our Edgewood location.
Core Responsibilities- Telephone support and triage for patients, scheduling problem visits or nurse visits when appropriate.
- Offering symptom management, supporting refill requests, and answering visit follow‑up questions.
- Providing patient education, particularly around chronic conditions, in person, over the phone or in group settings.
- Monitoring and coordinating the delivery of the care plan.
- Forming relationships with patients and their caregivers to support prevention‑focused care and ED/hospital diversion when appropriate.
- Providing direction to medical assistants to support delivery of procedures, telephonic support, and patient instructions.
- Conducting medication and testing authorizations.
- Delivering an exceptional patient experience through service, responsiveness, and care.
- Periodically performing home visits as needed.
- Other duties as assigned.
- Current state license to practice as a Registered Nurse.
- CPR certification required.
- Experience with EMR systems.
- Strong communication skills and a customer‑service orientation.
- Problem‑solving orientation, accountability, flexibility, and a positive attitude.
- Supportive attitude toward an older adult patient population.
- At least 2 years of RN experience, preferably in a primary care office or other outpatient setting.
- Fluency in Spanish, Polish, Russian, or other languages spoken by the communities served (where necessary).
- U.S. work authorization.
