Job Description:

The Social Worker Case Manager will train for Inpatient and work in various units including ICU, PCU, Med Surg, and ER. This role requires rotational weekend shifts and may include providing remote support to Off Campus Emergency Departments. Responsibilities:
Perform behavioral health assessments, diagnoses, and treatment planning Manage discharge planning for post-acute placements Provide acute care case management Requirements:
