Overview

This is a unique opportunity to shape the future of value-based care. DaVita is at the forefront of transforming kidney care, and we are looking for an exceptional operational leader to join our Integrated Kidney Care (IKC) team. As a Market Director for our Chronic Kidney Disease (CKD) program, you will play a pivotal role in managing a complex market, leading a dedicated team to improve patient outcomes and drive program success, and fostering critical partnerships with our nephrologist and DaVita Kidney Care partners.

Key objectives include providing upstream education and preventative care to delay CKD disease progression, supporting patients with comprehensive care and resource navigation to manage all their health needs, addressing root-causes to avoid preventable hospital readmissions, and ensuring patients are set up for smooth transitions when/if they transition to dialysis.

This is a hybrid field position that requires local and regional travel (at minimum 1x monthly).

Core Responsibilities
  • Lead and develop a diverse team of managers, who lead frontline clinical teammates, including Nurse Practitioners (NPs), Registered Nurses (RNs), and Care Coordinators (CCs).
  • Drive all aspects of talent management, including recruiting, hiring, onboarding, performance management, and development.
  • Create and maintain consistent and high quality team management processes to build and retain a stable, high-performing team.
  • Support strong team communication, a proactive solutions orientation, and a culture that embraces change.
  • Oversee day-to-day operations to achieve key clinical and financial goals.
  • Utilize performance dashboards to monitor key metrics, identify opportunities, and proactively develop action plans to ensure continuous improvement.
  • Create and maintain consistent and high quality metrics management processes to drive results and achieve goals.
  • Build and maintain strong, collaborative relationships with nephrologist partners, and proactively address barriers as they arise.
  • Serve as the key point of contact for IKC in your market, representing the value of the program and ensuring partner confidence.
What You'll Bring to the Team
  • Proven experience in operational leadership, preferably in a value-based care or clinical healthcare setting.
  • Strong talent management skills with experience hiring, developing, and retaining top talent.
  • Demonstrated ability to establish clear processes, drive accountability, and consistently achieve performance metrics.
  • Excellent relationship management skills, with the ability to build trust and collaborate effectively with physicians and other key partners.
  • Experience managing complex market dynamics such as growth, new program/pilot launches, ever-evolving technology, processes or team structure.
  • Ability to work in a hybrid model, with availability for local travel to physician offices, as well as regional travel for quarterly team meetings (in locations like Portland, Seattle, or Sacramento) or leadership meetings (in locations like Denver).
  • A passion for patient care and a commitment to living DaVita's core values.
Additional Requirements
  • Bachelor’s degree in related area, Masters Preferred.
  • Minimum of six (6) years’ supervisory experience.
  • Intermediate computer skills and proficiency in MS Word, Excel, PowerPoint, and Outlook.
  • Deals with confidential information and/or issues using discretion and judgment.
