RN Case Manager

Triniti Home Health & Hospice is hiring a RN Case Manager to join our team. This full‑time, benefits‑eligible position is based at our South Metro Branch in Edina.

• Position Type: Full-Time, benefits‑eligible
• Location: 4600 W. 77th Street, Edina, MN
• Wage Range: $42 – $45 per hour, depending on experience

• Assess the needs of home care patients, develop and implement personalized care plans, and execute interventions to help patients meet goals.
• Promote restoration of health through teaching, counseling, and preventive, rehabilitative, and palliative measures.
• Coordinate, oversee, and provide patient‑related care.
• Document and maintain paperwork, referral records, and reports.
• Display a courteous attitude and respect for all patients, families, and staff.
• Perform other duties as assigned.

• Current MN Registered Nurse (RN) license required.
• 1+ year of prior RN experience required.
• Proficiency with MS Word, Excel, and Outlook.
• Strong written and oral communication skills and ability to collaborate with care team members.
• Ability to organize and prioritize work efficiently to accomplish tasks timely.
