Triniti Home Health & Hospice is hiring a RN/LPN Visit Nurse – Hospice to join our compassionate team. Position Type: Full-Time, benefit-eligible position based on our West Metro area.
Wage Range: $32 - $44 /hour depending on licensure and experience.
RN/LPN Visit Nurse Responsibilities:- Provide skilled nursing care to hospice patients according to individualized care plans and physician orders.
- Assess patients' health status, monitor vital signs, and report changes in condition to the RN Case Manager.
- Collaborate with physicians, families, and the interdisciplinary care team to coordinate quality patient care.
- Obtain and implement physician orders and reorder medications as needed.
- Complete accurate and timely electronic documentation, with the majority of documentation completed at the point of care.
- Schedule and complete patient visits during regular business hours and participate in weekend/holiday rotation as assigned.
- Participate in case conferences, interdisciplinary team meetings, staff meetings, and ongoing educational opportunities.
- Maintain compliance with all state and federal home health and hospice regulations.
- Perform other duties as assigned.
Qualifications:
- Current RN or LPN license in the State of Minnesota.
- Minimum of 2 years of experience in geriatrics, home care, hospice, or a related healthcare setting.
- Strong knowledge of nursing practices and principles.
- Excellent assessment, communication, and interpersonal skills.
- Ability to establish positive relationships with patients, families, physicians, and interdisciplinary team members.
- Strong organizational and documentation skills with proficiency using electronic medical records.
- Ability to exercise sound clinical judgment while maintaining compassion and professionalism.
- Knowledge of HIPAA guidelines and commitment to maintaining patient confidentiality.
