Job Overview
Join M Health Fairview Southdale Hosptial and be part of Minnesota’s first EmPATH model of care, an innovative approach to mental health and substance use crisis care. In this role, you’ll provide emergency and urgent care in a supportive, living-room-style environment focused on stabilization, connection, and recovery. You’ll collaborate with an interdisciplinary team to assess patients, develop individualized care plans, administer medications, manage crises, and support safe transitions to the appropriate level of care.
Using trauma-informed care, therapeutic communication, de-escalation, and crisis intervention skills, you’ll help create a safe, therapeutic environment where patients can receive timely, compassionate care—and, when appropriate, avoid inpatient admission.
Provides Emergency/Urgent care in the EmPATH unit to the patient presenting with mental health or substance use symptoms / illness(s).
- Uses a detailed process to assess the physical and mental health needs of the patient, which will provide a framework from which patient care is provided in an observation setting. Knowledge of disease processes, therapy, complications, and encouraged outcomes for specific patient population(s).
- Contributes to the development of an individualized treatment plan in collaboration with the inter-disciplinary team providing a safe transition to appropriate level of care (e.g. community outpatient care, inpatient care).
- With the interdisciplinary treatment team, assesses needs for safe transition to community setting. If community setting not possible, supports transition to inpatient.
- Administers medications according to policy used for the management of behavioral patients with knowledge of actions, contraindications, side effects and desired outcomes
- Actively intervenes and to meet the identified emotional, psychological and physical needs, making revisions in care based on these outcomes.
Builds and maintains a therapeutic environment/milieu.
- Creates a therapeutic environment through active involvement and demonstrated response to change in patient symptoms and behavior
- Regularly assesses safety and suicide risk.
- Uses therapeutic communication skills and follows the parameters of the therapeutic relationship (e.g. trauma informed care, illness management and recovery, crisis intervention)
- Directs all emergency and crisis intervention care using established encouraging integrative therapies, de-escalation skills, behavioral emergency response techniques, and medications.
- Application of established policies and procedures regarding the legal aspects of providing care for EmPath / Mental Health and Addiction Services patients (ie:
Guardianship, Minnesota Statutes Chapter 253B Civil Commitment (72 hour hold, Health Officer Authority, Jarvis, Price);
Centers for Medicare and Medicaid Services (CMS), The Joint Commission (TJC), etc).
This position is 72 hours every two weeks on the day/evening shift, 12-hour shifts with every third weekend (Friday, Saturday and Sunday).
The Registered Nurse is responsible for the assessment, planning, delivery, and evaluation of nursing care for all assigned patient population(s). The RN is responsible for performing these responsibilities in accordance with all policy, procedures, professional practice guidelines and the MN Nurse Practice Act. The RN coordinates care with members of the health care team and delegates responsibilities to others as appropriate.
