POSITION SUMMARY:
The Community Outreach Coordinator for the Population Health Department is responsible for building and maintaining relationships with community partners, engaging with diverse populations, and promoting population health initiatives. This role involves developing and implementing outreach strategies to connect with the community, increase awareness of health resources, and support community health education. The Community Outreach Coordinator will report to the Population Health Officer and work closely with other team members to ensure the department's initiatives are effectively communicated and accessible to the community.
POSITION EDUCATION/
QUALIFICATIONS:
- High School Diploma or GED, required
- Bachelor's in communication, social work, healthcare administration, public health, or a related field (preferred)
- Bilingual Spanish and English (strongly preferred)
JOB KNOWLEDGE/
EXPERIENCE:
- 1 year of relevant healthcare experience (preferred)
- 2-3 years of experience in project coordination, healthcare, or a related field (preferred)
- Knowledge of community engagement strategies and population health principles.
- Strong interpersonal and communication skills, with the ability to engage diverse populations.
- Strong organizational skills and attention to detail.
- Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint)
- Excellent written and verbal communication skills.
Responsibilities:
POSITION RESPONSIBILITIES:
- Project Management:
Assist in the planning, coordination, and execution of population health projects, including but not limited to community outreach programs, health education initiatives, and data collection efforts. - Overseeing Daily Operations:
Support the Population Health Officer in overseeing daily operations of the department, ensuring smooth and efficient functioning. - Administrative Support:
Provide administrative support, including scheduling meetings, preparing reports, and maintaining project documentation. - Data Collection and Analysis:
Support data collection and analysis efforts to evaluate the effectiveness of population health initiatives and identify areas for improvement. - Communication and
Collaboration:
Facilitate communication between project teams, community partners, and other stakeholders. Ensure timely dissemination of project updates and outcomes. - Budget Monitoring:
Assist in monitoring project budgets and ensuring compliance with financial guidelines and requirements. - Event Coordination:
Organize and coordinate events, workshops, and training sessions related to population health initiatives. - Other Responsibilities as Necessary:
Take on additional duties as required to support the goals and objectives of the Population Health Department.
