We are seeking a full-time Family Medicine Nurse Practitioner (NP) or Physician Assistant (PA) to join a busy FM clinic with South Texas Health System Clinics in Weslaco, Texas. Rotating call coverage to support physicians - Patient Volume:
Diverse primary care patient population including Medicare and Medicaid patients - Location:
Weslaco, Texas (Rio Grande Valley) - Practice Setting:
Outpatient primary care within a multispecialty integrated health system - Support / Care Team:
Collaborative team including physicians, APPs, and managed care resources - EMR / Technology:
Electronic medical record system
- Provide comprehensive family medicine care in person and via telemedicine
- Support physicians with value‑based care initiatives and HEDIS measure completion
- Manage patient transitions from emergency room and inpatient discharge to outpatient follow‑up within 7 days
- Educate patients on proper utilization of emergency and urgent care services
- Collaborate with physicians to ensure accurate coding, risk adjustment, and quality documentation
- Participate in after‑hours and weekend call rotation
- Cover for physicians during leave periods
- Champion coordinated care models for Medicare and Medicaid populations

- Active or eligible Texas license
- Prior family medicine experience preferred
- Spanish fluency strongly preferred
