We are seeking a full-time Family Medicine Nurse Practitioner (NP) or Physician Assistant (PA) to join a busy FM clinic with South Texas Health System Clinics in Weslaco, Texas.   Standard clinic hours with on‑call rotation for after‑hours and weekends
• Call:
  Rotating call coverage to support physicians
• Patient Volume:
  Diverse primary care patient population including Medicare and Medicaid patients
• Location:
  
  Weslaco, Texas (Rio Grande Valley)
• Practice Setting:
  Outpatient primary care within a multispecialty integrated health system
• Support / Care Team:
  Collaborative team including physicians, APPs, and managed care resources
• EMR / Technology:
  Electronic medical record system

• Provide comprehensive family medicine care in person and via telemedicine
• Support physicians with value‑based care initiatives and HEDIS measure completion
• Manage patient transitions from emergency room and inpatient discharge to outpatient follow‑up within 7 days
• Educate patients on proper utilization of emergency and urgent care services
• Collaborate with physicians to ensure accurate coding, risk adjustment, and quality documentation
• Participate in after‑hours and weekend call rotation
• Cover for physicians during leave periods
• Champion coordinated care models for Medicare and Medicaid populations

• Licensed Nurse Practitioner or Physician Assistant with Board Certification in Family Medicine
• Active or eligible Texas license
• Prior family medicine experience preferred
• Spanish fluency strongly preferred
