POSITION SUMMARY

Assessment Coordinator shall be responsible for providing administrative support to the patient admission process. The incumbent shall also demonstrate accountability for and contribution to program development, quality improvement, problem solving and productivity enhancement in a flexible interdisciplinary fashion.

POSITION EDUCATION / QUALIFICATIONS
  • Graduate of accredited school of nursing required.
  • Current active registered nurse licensure in the State of Texas required.
  • Current BLS Certification required.
  • Valid Texas Driver’s License.
  • Willingness to be cross‑trained to other areas.
  • Organizational skills are necessary.
  • Good written and verbal communication skills.
  • Ability to read, write and speak English.
  • Ability to communicate clearly and concisely with all levels of nursing, administration, and physicians.
  • Prefer hospital experience.
JOB KNOWLEDGE / EXPERIENCE
  • Three to five years experience in a health care setting strongly preferred.
  • Requires strong communication and organizational skills.
  • Requires reasoning ability and good independent judgment.
  • Must present a professional image.
  • Requires working with frequent interruptions.
POSITION RESPONSIBILITIES
  • Promotes the facility mission, vision and values by effectively communicating them to others. Considers mission, vision and values in developing services, standards and practice.
  • Arrange for pre‑admission screening on. Verify screening arrangements within 30 minutes of receiving referral.
  • Assist in orienting patient and families to unit.
  • Pre‑admission screening completed according to policy & procedure.
  • Eligibility determination is given within one business day of referral. Alternate placement suggestions are offered if needed.
  • Complete verification of admission pre‑certification and insurance benefits if patient is to be admitted.
  • Coordinate bed availability with Nurse Manager / Program Director.
  • Develop network with case managers for referrals and as a resource to aid in discharge planning.
  • Communicates with hospital case managers daily regarding potential post acute needs of acute patients via phone, face to face, or e‑mail.
  • Conduct center tours for patients and families.
  • Attends team business meetings and offers ideas/solutions in problem solving situations/activities regarding the program.
  • Function as a resource for and provide team members with in‑service/cross‑training on admissions knowledge and skill areas.
  • Contributes to and cooperates with the implementation of system improvements.
  • Offers to assist other staff and demonstrates flexibility in carrying out shared team functions including special projects.
  • Completes tasks as requested.
  • Utilizes resources appropriately and uses/maintains equipment in order to prolong working life.
  • Attends scheduled unit quality management meetings as assigned.
  • Submits data as requested.
  • Assists with coverage for Case Management/Social Work as needed for inpatient and outpatient Rehab (if applicable).
  • Adheres to safety standards for staff and patients, and encourages compliance by others.
  • Other duties as assigned.

AGE SPECIFIC

Employees must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served in their assigned unit. The individual must demonstrate knowledge of principles of growth and development over the lifespan and possess the ability to assess data reflective of the patient’s status and interpret the appropriate information needed to identify each patient’s requirement relative to his or her age.
