Cardiac Transitional Care Advanced Practice Provider Nurse Practitioner Physician Assistant

The Cardiac Transitional Care Advanced Practice Provider utilizes a patient-centered coordinated care model, demonstrating competencies in leadership, direct clinical practice, consultation/collaboration, coaching/guiding, research, and ethical decision-making. The APN or PA works collaboratively with the practice/hospital team to assess, plan, and implement care for individuals with health and safety needs. The position will cover multiple cardiac service lines where needed (Observation, HF/MI, and Cath lab).

• Minimum of 3 years of experience in an acute care setting (hospital-based experience) required.
• Cardiology experience strongly preferred.
• Current APP licensure and certification (NP or PA) in good standing.
• Strong clinical assessment and decision‑making skills.
• Excellent communication skills, proven team player, ability to multitask and prioritize, and proactive and creative problem‑solver with a focus on process improvement, care coordination, and patient-centered solutions.

• Graduate of an NLN/AACN accredited nursing program.
• Master's or Doctorate degree from an accredited Nurse Practitioner program.
• Maintains clinical competencies through continuing education and clinical practice.
• Experience in a relevant patient care setting (or as regulations dictate).
• National Provider Identifier (NPI).
• Prescriptive authority through CDS and DEA registration according to New Jersey state law.
• Collaborative agreement with physicians on staff.
• Excellent written and verbal communication skills.
• Proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms.

• New Jersey Advance Practice Nurse License.
• BLS AHA Basic Health Care Life Support HCP Certification.
• Board certified by AANP or ANCC.

• Advanced Cardiac Life Support Certification.

  • Successful completion of a Physician Assistant program accredited by the AMA Council on Medical Education.

  • Current, unrestricted New Jersey Physician Assistant License.
  • Certification by the National Commission on Certification of Physician Assistants (NCCPA).
  • Current Basic Life Support (BLS) certification from the American Heart Association (AHA).
• Skills & Abilities:
  • Excellent written and verbal communication skills.
  • Proficiency with computer platforms, including Microsoft Office and/or Google Suite.

• Certifications:
  • Current Advanced Cardiac Life Support (ACLS) certification.
