Overview

FOX Rehabilitation is expanding in Morristown, NJ and seeks licensed Physical Therapists to provide exceptional one‑on‑one care to older adults in private residences and senior living communities. The role emphasizes autonomy, flexibility, and meaningful patient outcomes through the Geriatric House Calls™ model.

• Provide one‑on‑one physical therapy to older adults in private residences, senior living communities, independent living, assisted living, and memory care settings.
• Perform evaluations, develop individualized care plans, and deliver evidence‑based treatments.
• Conduct therapy sessions that promote patient wellness, longevity, mobility, and functional independence.
• Document all care accurately within FOX’s EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and fitness specialists.
• Educate patients, caregivers, and senior living staff on safety, functional performance, and aging in place.
• Provide telehealth or virtual care as appropriate to extend access and flexibility.

• Full‑time Physical Therapist – In‑Home & Senior Living Care (salaried with benefits).
• Part‑time Physical Therapist – In‑Home & Senior Living Care (salaried with benefits; eligibility based on minimum hours).
• PRN / FOX Flex Physical Therapist – Home & Community Visits (pay per unit compensation).

• Valid Physical Therapy license in the state(s) of practice or eligibility to obtain one.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

• Ability to lift 25–50 lbs occasionally and 10–25 lbs frequently; up to 10 lbs constant effort.
• Perform CPR.
• Provide moderate to maximum assistance with patient transfers and bed mobility.
• Reposition patients, stand, walk, climb stairs, and perform therapeutic activities for extended periods.
• Drive to patient homes and senior living communities.
• Wear PPE when required.
