Spring Creek Healthcare Center is seeking a highly motivated and compassionate Physical Therapist to join our dynamic team on a per diem basis. • As a Physical Therapist, you will work closely with patients to develop and implement individualized treatment plans designed to improve their physical function and mobility.
• You will assess patients' physical abilities and needs, and design and implement a variety of interventions to address these needs.
• You will work collaboratively with other members of the healthcare team to ensure that patients receive comprehensive and effective care.
• You will maintain accurate and up-to-date records of patient care, including treatment plans, progress notes, and discharge summaries.
• You will participate in quality improvement initiatives and contribute to the development of best practices in physical therapy.

• Conduct initial and ongoing assessments of patients' physical abilities and needs.
• Develop and implement individualized treatment plans, including goals, objectives, and interventions.
• Provide physical therapy interventions, including exercise programs, therapeutic activities, and education.
• Monitor and document patient progress, adjusting treatment plans as needed.
• Collaborate with other healthcare professionals to develop and implement comprehensive care plans.
• Participate in quality improvement initiatives and contribute to the development of best practices in physical therapy.

• We are expanding our post-acute programming in collaboration with area specialists and undergoing full-scale renovations to enhance our facilities and services.
