Physical Therapist (PT) – Full-Time

$46–$51/hour | Full-Time | Benefits
Ultimate Therapy is seeking a Physical Therapist (PT) to join the rehabilitation team at Autumn Lake s full-time opportunity is a great fit for a therapist who enjoys helping residents rebuild strength, improve mobility, and work toward greater independence through individualized rehabilitation.
What You'll Do- Evaluate residents and develop personalized physical therapy plans of care
- Provide skilled treatment focused on strength, balance, gait, transfers, endurance, and functional mobility
- Establish appropriate rehabilitation goals and track resident progress
- Adjust treatment plans based on clinical findings and individual needs
- Educate residents and caregivers on exercises, mobility strategies, and safe movement
- Complete accurate and timely clinical documentation
- Collaborate with PTAs, OT, SLP, nursing, and other members of the interdisciplinary care team
- $46–$51/hour
- Full-time position

- Active New Jersey Physical Therapist license required
- Graduate of an accredited Physical Therapy program
- Strong clinical, communication, and teamwork skills
- Skilled nursing or post-acute rehabilitation experience preferred, but not required
- New graduate Physical Therapists are welcome to apply
