Admissions Director

We are seeking an experienced and motivated Admissions Director to join our skilled nursing and rehabilitation team!

The Admissions Director will play a key role in managing the admissions process, building strong referral relationships, and helping ensure a smooth transition for residents and their families. We are looking for someone who is organized, personable, and understands the fast-paced admissions environment within a skilled nursing facility.

• Manage and coordinate the facility's admissions process from referral through admission
• Review referrals and work closely with clinical and administrative teams
• Communicate with hospitals, referral sources, residents, and families throughout the admissions process
• Build and maintain strong relationships with referral partners
• Coordinate admissions to help ensure a smooth transition into the facility
• Maintain accurate admission documentation and follow-up
• Work collaboratively with facility leadership to support census and occupancy goals

• Previous Admissions Director experience in a skilled nursing facility is required
• Strong knowledge of the skilled nursing admissions and referral process
• Excellent communication, organization, and relationship-building skills
• Ability to manage multiple referrals and priorities in a fast-paced environment
• Strong attention to detail and follow-through
• Team-oriented with a positive and professional approach
