As a Hospice LPN, you will:
- Deliver Direct Patient Care: Administer nursing care to hospice patients in accordance with their individualized care plans, focusing on comfort and symptom management.
- Collaborate with the Care Team: Work closely with registered nurses, physicians, and other healthcare professionals to evaluate and adjust care plans, ensuring comprehensive support for patients.
- Educate and Support Families: Provide guidance to patients families on care techniques, disease progression, and available resources, fostering a supportive environment.
- Maintain Accurate Documentation: Ensure all patient interactions, treatments, and observations are meticulously recorded in compliance with hospice, state, and federal guidelines.
- Participate in Continuous Improvement: Engage in team meetings and quality assessment activities to enhance the standard of care provided.
To excel in this role, you should have:
- Current LPN license to practice in NJ
- At least two years of nursing experience, preferably in hospice, home care, or related fields.
- Ability to effectively interact with patients, families, and the healthcare team, demonstrating empathy and clarity.
