Hackensack Meridian Health - Up to $22.35/hour - - Responsibilities:
Provide patient care per the Patient Care Plan, including ROM exercises, ambulation, feeding, vital signs, and daily tasks;
Document and report daily patient care activities per policy and state/federal regulations;
Identify and report real or potential problems in patient care to appropriate team members (RN/Charge Nurse);
Ensure residents use adaptive equipment as ordered and are clean, groomed, safe, and fed during shift;
Collaborate with multidisciplinary team and participate in committees as requested
