Deliver care and services for designated group of guests/residents according to Individualized Service Plans (ISP).;
Report all incidents immediately to appropriate supervisor / team member.;
Participate in the care planning for their designated guests/residents.;
Obtain, document and report the guest/residents vital signs, urine output and meal intake per individualized care plan to licensed nurse as requested.;
Assist in maintaining assistive devices, adaptive equipment and other medical equipment to include cleaning equipment and making readily available.
