Hackensack Meridian Health - - Responsibilities:
Provide direct resident care to ensure the resident's plan of care is followed;
Complete all required documentation per state/federal and facility guidelines;
Assist with ROM exercises, ambulation, transfer training, feeding and calorie counts, bed making, personal hygiene, vital signs, and daily reports;
Communicate pertinent information to RN and participate in multidisciplinary teamwork;
Foster professional relationships and participate in committees as requested by supervisors
