JOB JOB OVERVIEWThe Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN) is responsible for providing direct resident care and supervising direct resident care of non‑licensed team members in the skilled nursing operations and other areas throughout the community as appropriate. The role entails leading and demonstrating Sunrise Senior Living’s mission of “champion quality of life for all seniors,” in accordance with federal, state/provincial, and local laws, regulations, and company policies to promote the highest degree of quality care and services to our guests/residents.
RESPONSIBILITIES& QUALIFICATIONS
- Assist the Leadership Team in supervising nursing team members.
- Assist with the development and leadership of the Designated Nursing Assistants (DNA) / Certified Nursing Assistants (CNA) following the Resident Centered Care model.
- Provide clinical care including urinary catheterization, tube feedings, dressing changes, Accu‑checks, medication management/administration, and admission, transfer, and discharge of guests/residents.
- Complete timely and accurate documentation of resident charts.
- Complete Incident Reports and report to the Director of Nursing Services (DNS).
- Review, update, and document information in the electronic health record.
- Conduct effective visits/rounds at least twice a day; also visit/round with physicians as assigned by DNS.
- Research and document all incidents of abuse, neglect, and misappropriation of funds as assigned by DNS, Executive Director (ED), or Skilled Nursing Administrator (SNA).
- Participate in the Clinical IDT meeting and in the Daily Prospective Payment System (PPS) meetings to obtain information on admissions and discharges for the day/week.
- Assist in monitoring medication records and administration.
- Assist in receiving lab, x‑ray, and other test results and communicate appropriately.
- Strive for excellent quality care and service delivery as measured in the Quality Services Review (QSR) process and implement standard practices and excellence in resident care.
- Demonstrate knowledge in key quality improvement areas such as the Skilled Nursing Transformation and Resident Centered Care Model, Sunrise and Federal Quality Indicators, and participate in Sunrise Quality Indicator Meetings.
- Demonstrate knowledge of key regulations including all federal, state/provincial, and local resident care nursing regulations, resident rights, the Resident Assessment Instrument (RAI) process, and actively participate in federal, state/provincial, and local surveys.
- Follow documentation guidelines of the Restorative process and train and monitor DNAs/CNAs in the Restorative program.
- Demonstrate Sunrise practices and guidelines for skin, nutrition, behavior, falls, and bowel/bladder care; assist in implementation, training, and monitoring of documentation related to clinical systems weekly; consult with physicians as appropriate; ensure concerns are reported to DNS.
- Transcribe physician orders to the Medication Administration Record/Treatment Administration Record (MAR/TAR) accurately and promptly; demonstrate the Six (6) Rights of Medication Pass.
- Perform Sunrise and nursing standards for taking orders from physicians and administering medications; ensure current resident photo with dates for all residents on MAR/TAR; complete Med Pass audit quarterly and MAR/TAR audit monthly; monitor medication rooms and carts for neatness, cleanliness, availability, and expiry.
- Respond to resident, family, and visitor requests timely and professionally; communicate with families regarding resident medical care and incorporate requests into nursing standards; maintain confidentiality; review, read, and initial the Daily Log.
- Partner with community team to ensure adherence to national/provincial regulations for occupational health and safety; practice safety procedures including PPE, fire extinguishers, Safety Data Sheets, and Lockout‑Tagout.
