Purpose and Scope
The professional Registered Nurse Outpatient RN CAP 2 may be an entry-level designation into the Clinical Advancement Program (CAP) for new employees who meet the RN CAP 2 criteria or are advanced from RN CAP 1. This position is accountable and responsible for the provision and coordination of clinically competent care—including assessment, planning, intervention, and evaluation—for an assigned group of patients.
It may include delegation of appropriate tasks to direct patient care staff, including RNs, LVN/LPNs, and Patient Care Technicians.
As a member of the End Stage Kidney Disease (ESKD) health care team, this position participates in decision-making, teaching, leadership functions, and quality improvement activities that enhance patient care outcomes and facility operations.
Principal Duties and Responsibilities- Perform all essential functions under the direction of the Supervisor and with guidance from the Clinical Educator, Preceptor, or in collaboration with other Registered Nurses.
- Collect and analyze patient data pre‑, during‑, and post‑hemodialysis treatment for assigned patients and document in the patient medical record.
- Make adjustments or modifications to the treatment plan as indicated and notify Team Leader, Charge Nurse, Supervisor, or Physician as needed.
- Asses, collaborate with, and document patient/family learning needs; provide initial and ongoing education.
- Direct and provide, in collaboration with direct and ancillary patient care staff, all aspects of the daily safe and effective delivery of hemodialysis therapy to assigned patients.
- Administer medications as prescribed or per approved algorithm(s), documenting justification and effectiveness.
- Assist or initiate emergency response measures.
- Serve as a resource, leader, coach, mentor, and role model for new and incumbent employees.
- Ensure correct laboratory collection, processing, and shipping procedures; reschedule missed or insufficient collections.
- Identify expected outcomes and update the nursing assessment and plan of care for assigned patients in collaboration with the Interdisciplinary Team.
- Ensure patient awareness of transplant and treatment modality options.
- Participate in education and quality improvement projects at facility and area level as directed by Supervisor.
- May serve as a Preceptor to new employees and complete CAP requirements as needed.
The position involves heavy lifting, moving patients, and assisting with ambulation. Employees must stand for prolonged periods, bend over, and may need assistance moving equipment up to 200 lbs. and lifting chemical and water solutions up to 30 lbs. The work environment is a health care facility with controlled air temperature and moderate noise levels. Exposure to infectious diseases/materials is possible.
The role may require travel to training sites or other facilities.
Oversight of Patient Care Technicians, LVN/LPNs, and RNs as a Team Leader or designated Nurse in Charge after completing all FKC education and training requirements; minimum of 3 months experience in chronic/acute hemodialysis as an RN.
Education and Licensure- Graduate of an accredited School of Nursing.
- Current appropriate state licensure.
- Current or successful completion of CPR BLS Certification.
- Must meet practice requirements in the state of employment.
- Entry level for RNs with a minimum of 2 years of nephrology nursing experience.
Hourly Rate: $37.00 – $63.00
