Overview

We are seeking a dedicated and compassionate Hospice/Palliative Care Nurse Practitioner (NP) to join our client’s interdisciplinary team, providing holistic care to patients with serious or life‑limiting illnesses. The ideal candidate will have experience in palliative care or hospice, demonstrating expertise in symptom management, emotional support, and end‑of‑life care planning. This role involves working collaboratively with physicians, nurses, social workers and other team members to ensure the highest quality of patient‑centered care.

Key Responsibilities
  • Perform comprehensive assessments of patients with chronic, advanced, or terminal illnesses, including physical, emotional, social, and spiritual needs.
  • Develop and implement individualized care plans focused on symptom management, quality of life, and patient goals.
  • Provide pain and symptom management, including prescribing medications and treatments in accordance with state regulations.
  • Facilitate discussions about advance care planning, goals of care, and end‑of‑life decisions with patients and families.
  • Collaborate with the interdisciplinary palliative care team to ensure a coordinated approach to patient care.
  • Serve as a resource and consultant to other healthcare providers regarding palliative care principles and practices.
  • Provide education and emotional support to patients’ families and caregivers, helping them navigate the complexities of serious illness and end‑of‑life care.
  • Act as a liaison between patients, families, and the healthcare team to address concerns and ensure understanding of care plans.
  • Educate patients, families, and staff on palliative care principles, including symptom management and advance care planning.
  • Advocate for patients’ needs and preferences within the healthcare system.
Requirements
  • Master’s degree in Nursing with certification as a Nurse Practitioner (NP).
  • Current NP licensure in New Jersey.
  • Minimum of 1 year of experience in palliative care or hospice.
  • Spanish‑speaking abilities preferred but not required.
  • Excellent communication and interpersonal skills, with the ability to have sensitive conversations about serious illness and end‑of‑life care.
  • Strong clinical judgment and critical thinking skills.
  • Ability to work collaboratively within a team and independently when needed.
  • Empathy, compassion, and a commitment to patient‑centered care.
Benefits

The annual base salary range is $130,000 to $165,000. Actual compensation offered to the successful candidate may vary based on geographic location, work experience, education, and skill level. Bonus compensation details, if applicable, will be finalized at the time of offer.
