A day in the life of a Licensed Practical Nurse at Hackensack Meridian Health includes:

• Supervises and assigns CNAs on the unit.
• Follows the nursing care plan and utilizes nursing diagnoses in cooperation with the supervisor and unit personnel.
• Follows the plan of care for residents, consistently, accurately, and independently prioritizing and completing routine assignments within guidelines and schedules.
• Recognizes psychosocial needs, their possible effects on behavior, and intervenes to provide emotional support.
• Maintains accurate records of all patients in the unit.
• Identifies and reports all real or potential problems regarding the development or administration of patient care.
• Records, reports, and interprets patient responses to treatment and care to appropriate members of the inter‑disciplinary team.
• Identifies changes in patient conditions/needs and reports to charge nurse.
• Demonstrates a team approach to problem solving through frequent communication and collaboration on complex issues to provide appropriate care.
• Maintains all equipment as per facility policy and uses equipment consistent with specifications.
• Participates and represents Harborage and Nursing staff on committees as requested by supervisors.
• Identifies patient needs and treatment opportunities, providing services to meet those needs on an ongoing basis.
• Maintains professional competence and stays current with occupational health trends through continuing education, in‑service programs, professional staff sessions, and training of nursing personnel.
• Fosters good relationships and communications with all team members, patients, families, and visitors.
• Assists with new team member orientation and provides key experiences to prepare teammates for independent performance of essential job functions.
• Reports problems to Manager promptly.
• Other duties and/or projects as assigned.
• Adheres to HMH organizational competencies and standards of behavior.

• Completion of an approved LPN program.
• Current with all continuing education (CE) requirements.
• Perform clinical care professionally.
• Serve as patient advocate and role model to other team members.
• Deliver patient care with minimal disruptions and obstacles.
• Ability to effectively work on a multidisciplinary team.
• Excellent written and verbal communication skills.
• Proficient computer skills, including Microsoft Office and/or Google Suite platforms.
• Endurance to stand and walk for extended periods of time.
• Ability to lift, push, and pull a minimum of 50 pounds.

• Long‑term care facility experience.
• Prior work experience with geriatric and disabled populations.
• Bi‑lingual (Spanish).

• Licensed Practical Nurse.
• BLS AHA Basic Health Care Life Support.
• HCP Certification within 60 days of entering position.

• IV Certification.

Minimum rate of $32.97 hourly.
