ResponsibilitiesA day in the life of a Licensed Practical Nurse at Hackensack Meridian Health includes:
- Supervises and assigns CNAs on the unit.
- Follows the nursing care plan and utilizes nursing diagnoses in cooperation with the supervisor and unit personnel.
- Follows the plan of care for residents, consistently, accurately, and independently prioritizing and completing routine assignments within guidelines and schedules.
- Recognizes psychosocial needs, their possible effects on behavior, and intervenes to provide emotional support.
- Maintains accurate records of all patients in the unit.
- Identifies and reports all real or potential problems regarding the development or administration of patient care.
- Records, reports, and interprets patient responses to treatment and care to appropriate members of the inter‑disciplinary team.
- Identifies changes in patient conditions/needs and reports to charge nurse.
- Demonstrates a team approach to problem solving through frequent communication and collaboration on complex issues to provide appropriate care.
- Maintains all equipment as per facility policy and uses equipment consistent with specifications.
- Participates and represents Harborage and Nursing staff on committees as requested by supervisors.
- Identifies patient needs and treatment opportunities, providing services to meet those needs on an ongoing basis.
- Maintains professional competence and stays current with occupational health trends through continuing education, in‑service programs, professional staff sessions, and training of nursing personnel.
- Fosters good relationships and communications with all team members, patients, families, and visitors.
- Assists with new team member orientation and provides key experiences to prepare teammates for independent performance of essential job functions.
- Reports problems to Manager promptly.
- Other duties and/or projects as assigned.
- Adheres to HMH organizational competencies and standards of behavior.
- Completion of an approved LPN program.
- Current with all continuing education (CE) requirements.
- Perform clinical care professionally.
- Serve as patient advocate and role model to other team members.
- Deliver patient care with minimal disruptions and obstacles.
- Ability to effectively work on a multidisciplinary team.
- Excellent written and verbal communication skills.
- Proficient computer skills, including Microsoft Office and/or Google Suite platforms.
- Endurance to stand and walk for extended periods of time.
- Ability to lift, push, and pull a minimum of 50 pounds.
- Long‑term care facility experience.
- Prior work experience with geriatric and disabled populations.
- Bi‑lingual (Spanish).
- Licensed Practical Nurse.
- BLS AHA Basic Health Care Life Support.
- HCP Certification within 60 days of entering position.
- IV Certification.
Minimum rate of $32.97 hourly.
