LPN / LVN Position – Sunrise Senior Living, Edison, NJ

Responsibilities& Qualifications Clinical Care
Assist the Leadership Team in supervising nursing team members; assist in developing and leading DNAs/CNAs following the Resident‑Centered Care model; provide clinical care including urinary catheterization, tube feedings, dressing changes, Accu‑check, medication management/administration, admissions, transfers, and discharges; complete timely chart documentation; submit incident reports to the Director of Nursing Services; conduct effective visits/rounds twice a day; research and document incidents of abuse, neglect, and misappropriation;
participate in Clinical IDT and PPS meetings; monitor medication records; receive and communicate lab, X‑ray, and other test results.
Maintain excellent quality care measured by the QSR process; demonstrate knowledge of Skilled Nursing Transformation, Resident‑Centered Care Model, and Sunrise and Federal Quality Indicators; participate in Quality Indicator Meetings; develop quality improvement plans; keep abreast of federal, state, provincial, and local nursing regulations; engage in Resident Assessment Instrument process; participate in surveys; follow the Restorative process guidelines; train and monitor DNAs/CNAs in the Restorative program;
monitor documentation and refer residents as needed.
Demonstrate Sunrise practices for skin, nutrition, behavior, falls, and bowel/bladder; assist in implementation, training, and monitoring of clinical system documentation; consult with physicians; report changes, concerns, or solutions to the DNS.
Medication ManagementTranscribe physician orders into the MAR/TAR with accuracy; apply the Six Rights of Medication Pass; administer medications following Sunrise and nursing standards; maintain current resident photos on MAR/TAR; complete Med Pass audits quarterly and MAR/TAR audits monthly; monitor medication rooms and carts for cleanliness and correctness.
Guest/Resident, Family, and Visitor RelationsRespond to requests in a timely and professional manner; communicate with families about medical care; maintain confidentiality; review the Daily Log for resident changes.
Risk Management and General SafetyPartner with the community team to follow occupational health and safety regulations; promote risk management programs; practice safety procedures including PPE, fire extinguishers, SDS, and lockout‑tagout; support infection control training; handle exposure incidents; ensure oxygen tanks are stored properly.
SupervisionAssist with staffing and scheduling; provide evaluation feedback, coaching, and counseling; lead staff during orientation, trainings, and evaluations.
Training and Team SuccessParticipate in team goals; demonstrate the Team Member Credo; contribute to engagement programs; attend regular meetings; maintain compliance with required training and state/regulatory training; perform other duties as assigned.
