Registered Nurse; RN - Home Health Admission - per visit

BAYADA Home Health Care is seeking a Registered Nurse (RN) for Home Health Admissions in Edison, New Jersey. This role supports patients across Monmouth, Middlesex, Somerset, and Union counties by welcoming them into home health services, completing Start of Care assessments, and developing individualized care plans.

• Complete 1–3 Start of Care (SOC) visits each day.
• Conduct comprehensive patient assessments and accurately complete OASIS documentation.
• Develop individualized plans of care in collaboration with physicians and the interdisciplinary team.
• Deliver skilled nursing care, including medication administration, wound care, treatments, and patient education.
• Educate patients and caregivers on disease management, medications, and home safety.
• Monitor patient conditions and communicate changes appropriately.
• Ensure documentation is accurate, timely, and compliant with regulatory standards.

• Current RN license in the State of New Jersey.
• Minimum of one year of nursing experience (home‑health experience preferred).
• Experience completing OASIS documentation and Start of Care visits strongly preferred.
• Comfortable using electronic medical records (EMR);
  Homecare Homebase (HCHB) experience is a plus.
• Excellent assessment, communication, and time‑management skills.
• Passion for providing exceptional, patient‑centered home health care.

• Base Pay: $65.00 – $75.00 per point (annual earning potential $101,400 – $117,000).
• Sign‑On Bonus: $20,000 for Full‑Time RNs in Monmouth County only.
