Role Overview We are seeking a Wound Care Nurse Practitioner (NP) to join our growing team. This role is ideal for a clinician who thrives in autonomous settings, values patient-centered care, and is passionate about improving outcomes for individuals with acute and chronic wounds.

Responsibilities Perform comprehensive wound assessments and develop individualized treatment plans

Provide advanced wound care, including debridement, dressing changes, and infection management

Collaborate with physicians, caregivers, and facility staff to coordinate care Document visits and treatment plans accurately in EMR systems

Educate patients and caregivers on wound prevention and care Travel to patient locations within Livingston and nearby communities

Qualifications

Active Nurse Practitioner license in New Jersey Certification in Wound Care (WOCN, WCC, or equivalent) preferred

Minimum 1 year of wound care experience (home health or facility-based a plus)
Strong clinical judgment and ability to work independently

Excellent communication and documentation skills

Valid driver’s license and reliable transportation
