A fast-growing long-term care organization is seeking acompassionate and dedicated Nurse Practitioner to join itsteam at a nursing home in Edison,NJ. This is an excellent opportunity for an experienced provider who is passionate about delivering high-quality primary care to a geriatric patient population in asupportive and collaborative environment.

Responsibilities:

Provide comprehensive primary care services to nursing home residents
Performnew admission evaluations and ongoing follow-up visits
Monitor,assess, and manage acute and chronic medical conditions
Collaborate with interdisciplinary care teams to ensure optimal patient outcomes
Document patient encounters thoroughly and accurately in the medical record
Supportcontinuity of care and contribute to overall quality of care within the facility

Requirements:

Active Nurse Practitioner license in the state of New York
Active DEA license
Minimum6months of experience working as a Nurse Practitioner
Priornursinghome or long-term care experience strongly preferred
Highly competitive salary: $150,000–$155,000 annually
