The UVMMC Home Infusion Nurse Coordinator is responsible for ensuring a safe and timely transition of patients to home infusion. You will create, revise and assess the care plan, teach patients, caregivers and home health nurses to administer infusion therapy safely at home, and advise physicians and case managers on patient safety.
