Registered Nurse

• Create and manage plans of care for individual patients
• Oversee Licensed Practice Nurses (LPN)
• Create plans for LPNs to follow in caring for individuals
• Conduct check-ins with individuals at the group home
• Provide necessary medical care and monitor and follow through on care
• Provide medication education and related support
• Encourage normalization and independence of individuals served
• Provide training and assistance to individuals served and staff on medical education and related areas
• Liaise with outside medical providers to coordinate care
• Assist with medication administration and required documentation
• Maintain accurate written records regarding medical, dental, and related matters
• Intervene in emergencies when needed
• Attend interdisciplinary team (IDT) meetings
• Review medication logs, doctors’ appointments, follow-ups, and staff training

• Currently Licensed Registered Nurse (RN) in NJ
• Excellent interpersonal and client care skills
• Valid driver’s license
• First aid/CPR certification required
• Ability to build rapport with patients
• Experience working with individuals with developmental disabilities preferred

Demonstrates expertise in creating and managing individualized care plans, overseeing nursing staff, and providing medical education and support. Proficient in coordinating care with external providers and maintaining accurate medical records while ensuring patient independence and normalization.
