Community Liaison

Holistic are Hospice is seeking a self‑driven, compassionate Community Liaison to join our team serving the Red Bank area. In this role you will build trust‑based relationships with physicians, referral sources, and other healthcare partners, helping families access hospice care and driving census growth.

• Establish and nurture relationships with key decision‑makers, including physicians and healthcare partners.
• Advocate for our hospice services through regular outreach and visits.
• Position our hospice as the provider of choice within the service area, helping exceed growth and census goals.
• Drive referral growth through scheduled visits with referring physicians, hospital discharge planners, skilled nursing facilities, social workers, and other professionals.
• Support patients and families during critical moments by leading initial conversations about hospice care, explaining services and philosophy, and facilitating consent with empathy and respect.

• Bachelor’s degree in a related field preferred; additional experience may substitute for the educational requirement.
• Minimum two years of sales/marketing experience in healthcare required; hospice/palliative marketing experience strongly preferred.
• Proficiency in establishing and maintaining relationships with clinical staff and marketing partners.
• Proven ability to build and maintain new and existing accounts with physicians, healthcare professionals, and community agencies.
• Prior clinical experience is a plus.
• Reliable transportation, valid driver’s license, and auto insurance.

$80,000 – $98,000 per year (Average Base Pay Range) plus uncapped commission. Pay will be determined based on experience and responsibilities and confirmed during the interview process.
