Tuition Reimbursement Pastoral Care Services Free staff lunch every Friday Position Summary The Case Manager coordinates the implementation of Individual Employment Plans (IEPs). This role assesses and monitors the implementation and effectiveness of clinical interventions, conducts individual and group therapeutic sessions with program participants, and coordinates behavior management. The Case Manager discusses areas affecting program participants with related health services, DDD case managers, DVR counselors, Support Coordinators, and families/legal guardians/group home staff.
This role fosters input in communications, is aware of HIPAA regarding Personal Health Information (PHI) and recognizes that the highest quality of service is delivered with Hospitality, Respect, Quality, Spirituality, and Responsibility, the core values of the Agency, and maintains confidentiality of departmental records, persons, and activities.

Essential Duties and Responsibilities:
Provides case management services for consumers and their families and provides vocational rehabilitation counseling on an as-needed basis.
Writes and implements rehab plans, recommends, and tracks internal and external referrals, and records significant information for case entries.
Attends ISP/DVRS/DDD/DOL meetings as needed.
Completes quarterly production rates on a timely basis.
Monitors clients progress through record keeping and communicates with other community agencies (DVR, DDD, medical personnel, etc.)Provides client referrals to job development.
Promotes and facilitates self-advocacy among clients in cooperation with SMARTS (Self-Advocates Are Ready to Speak Up).Supports clients in community employment transitions.
Complies with standards set forth in DVR contracts, the CARF manual, and DOL regulations.
Prepares and keeps charts up to date for DVRS audits by PPDS (Program Planning and Development Specialist).Attends bi-weekly staff meetings and Clinical Case Conferences to presents updates and develops IEPs (Individual Employment Plans) including, but not limited to, reporting on client production rates and earnings/calculations.
Reviews referral profiles and maintains appropriate paperwork including case record documentation and billing reports.
Assists in hybrid, CBWE, and other community-based services with job coaching and case management when needed.
