Optimum Post Acute Care, PLLC is seeking a compassionate physician from any medical specialty to provide high-quality care to patients in skilled nursing facilities, rehabilitation centers, and other post-acute settings.

• Provide medical care in skilled nursing facilities, rehabilitation centers, and other post-acute settings
• Complete initial assessments, routine follow-up visits, and acute visits as needed
• Manage chronic conditions and changes in patients’ health
• Perform medication reconciliation and develop individualized care plans following hospital discharge
• Collaborate with nurses, therapists, case managers, specialists, and facility staff
• Communicate treatment plans and goals of care with patients and their families
• Maintain accurate and timely electronic health record documentation
• Support efforts to reduce hospital readmissions and improve patient outcomes

• Physician from any medical specialty
• Licensed or eligible for medical licensure in Oklahoma
• Comfortable managing medically complex patients
• Strong clinical judgment and decision-making abilities
• Excellent communication and interpersonal skills
• Able to work independently and collaborate with a multidisciplinary team
• Organized and committed to timely documentation

Full-time equivalent: $250,000–$280,000 annually

Part-time compensation will be prorated and determined according to the agreed-upon schedule, experience, and patient responsibilities.
