Optimum Post Acute Care, PLLC is seeking a compassionate physician from any medical specialty to provide high-quality care to patients in skilled nursing facilities, rehabilitation centers, and other post-acute settings.
This flexible, part-time opportunity is ideal for a physician looking to supplement their current schedule while caring for medically complex patients and making a meaningful difference throughout their recovery.
- Provide medical care in skilled nursing facilities, rehabilitation centers, and other post-acute settings
- Complete initial assessments, routine follow-up visits, and acute visits as needed
- Manage chronic conditions and changes in patients’ health
- Perform medication reconciliation and develop individualized care plans following hospital discharge
- Collaborate with nurses, therapists, case managers, specialists, and facility staff
- Communicate treatment plans and goals of care with patients and their families
- Maintain accurate and timely electronic health record documentation
- Support efforts to reduce hospital readmissions and improve patient outcomes
- Physician from any medical specialty
- Licensed or eligible for medical licensure in Oklahoma
- Comfortable managing medically complex patients
- Strong clinical judgment and decision-making abilities
- Excellent communication and interpersonal skills
- Able to work independently and collaborate with a multidisciplinary team
- Organized and committed to timely documentation
Full-time equivalent: $250,000–$280,000 annually
Part-time compensation will be prorated and determined according to the agreed-upon schedule, experience, and patient responsibilities.
