Touchmark at Coffee Creek is looking for the right person to join our team as our Clinical Care Manager. As the Clinical Care Manager, you will enrich lives every day by utilizing your education and experience to oversee the licensed care staff, ensuring residents receive the highest level of dignity and care. You will have the daily opportunity to distinguish yourself and your team of caregivers by taking the time to be a positive presence in the lives of residents and their family members!

Some Of The Things You Will Be Responsible For
  • Assess/coordinate all resident clinical care scope; quality assurance and regulatory oversight.
  • Manage Nurses and Medication Aides
  • Train, delegate to, and coordinate care amongst all nurses and medication aides.
  • Serves as primary point of contact for clinical provider communication.
  • In coordination with the Health Services Director, Executive Director, nursing staff, and sales team, perform pre-assessments, assist with move-in process to ensure smooth and timely move-in of new residents
  • Coordinate development of a life enrichment plan for each resident based on remaining strengths and abilities, as well as resident interests, skills, social and restorative goals, personal care requirements and physician recommendations
  • Assist Health Services Director with staff scheduling, recruitment, onboarding and ongoing training
The Ideal Candidate Will
  • RN degree from an accredited school of nursing and current state license
  • 4-6 years of experience working with older adults in a health care setting; geriatric mental health a plus
  • 3+ years of experience supervising staff
  • Have a genuine interest in caring for elderly residents and working in a geriatric environment
  • Possess a willingness to continue learning and improving skills, as well as a commitment to confidentiality
  • Be able to maintain patient and tactful composure when dealing with residents, family members, staff members and visitors
