North Care seeks a Behavioral Health Case Manager II to deliver integrated care across physical and behavioral health for clients and families. You will work with a multidisciplinary team to plan goals, coordinate services, and support treatment adherence while meeting ODMHSAS CM II standards.
You will conduct assessments, develop care plans, assist with benefits, housing and transportation, and ensure timely documentation. A valid OK driver’s license and ongoing certification are required.
