Family Peer Support Provider Home and Community Based Level 1

Position will advocate for families in the program, provide linkage with needed community resources, assist in treatment and education to families, and provide crisis support and safety planning under the clinical supervision of licensed staff.

• Supports implementation of the phases and activities of a unified system of care.
• Ensures the team moves children/youth and families toward long‑term stability through the treatment process.
• Provides coaching to children/youth and families to improve self‑efficacy.
• Respects the integrity and confidentiality of individuals receiving services.
• Supports development, reconnection, and strengthening of natural supports.
• Participates in treatment with the child/youth and family team to keep children/youth in the home and avoid out‑of‑home placements.
• Empowers the child/youth and family through advocacy, coaching, follow‑up, and support; teaches effective advocacy within child‑serving agencies such as DHS, Juvenile Justice, and the education system.
• Develops and maintains positive relationships with community partners; serves as liaison between Wraparound North Care, the Oklahoma County referral team, and other community services.
• Participates in community consultation and educational activities; promotes child/youth and family involvement at the individual and community levels.
• Carries a differentiated caseload involving highly complex situations requiring coordination of services through the wraparound process.
• Maintains records in a timely manner as required by agency, state or federal funding sources and accreditation authorities; completes all required forms and documents daily.
• Collects data for evaluation and research of program effectiveness and assists in obtaining non‑clinical information from families.
• Adheres to patient contact/caseload standards, responds promptly to phone calls, text pages, and email, and maintains consistent attendance.
• Participates in team meetings, clinical consultations, group and individual supervisions, and coaching.
• Attends trainings and workshops related to children’s mental health.
• Transports patients to community locations and provides services in the home.
• Completes all North Care required courses and training; abides by agency policy and program procedures.
• Performs other duties as required and/or assigned by supervisor and/or executive staff.

• Must possess a High School diploma or equivalent.
• Lived experience as a caregiver of a child who has navigated mental health services, DHS system, court system, or similar systems of care.
• Must obtain ODMHSAS Family Support Provider certification within 60 days of hire (or be certified) and complete annual continuing education.
• Must obtain ODMHSAS Wellness Coach Certification within 60 days of hire (or be certified) and complete annual continuing education.
• Must have a valid Oklahoma driver’s license and maintain automobile liability insurance, including uninsured motorist coverage.
• Must possess reliable transportation; local travel required; eligible for a phone and hotspot stipend.
• Must have CPR, First Aid, and Therapeutic Options certifications or obtain them within 30 days of employment.
• Must complete a TB test within 12 months prior to employment or within 30 days of hire.
• Must complete all North Care required training within 30 days of employment.
• Must be proficient in MS Word and possess intermediate general computer skills.
• Must have excellent written and oral communication skills and good interpersonal skills.
• Must be able to travel and self‑transport to client sites; must be physically able to move freely around facilities.
• Must dress in casual professional attire and adhere to the organization’s dress code.
• Community‑based: must work at least 20 hours per week in community settings and be available for travel and client appointments outside of the office.
