Care Manager MSWThe Clinical Social Worker partners with patients, families and the health care team to address and advocate for patients' social and emotional needs. The clinical social worker is responsible for providing a full range of social work services including psychosocial assessment, treatment planning, therapeutic interventions, discharge planning, crisis intervention, and resource referral. The role involves partnering with the patient and support system, and interdisciplinary teams both internal and external to the organization, to improve patient care through effective utilization and monitoring of health care resources.
The clinical social worker assumes a leadership role to facilitate achievement of patient goals and desired clinical, financial, and resource outcomes, guided by the NASW Code of Ethics and the Swedish Mission and Vision.
- Master's Degree in Social Work from an accredited school
- Upon hire:
Washington Clinical Independent Social Worker Associate License - 1 year of experience in acute medical or mental healthcare in an inpatient or outpatient setting
- ACM/CCM certification

Care Manager LCSWThe Clinical Social Worker partners with patients, families and the health care team to address and advocate for patients' social and emotional needs. The responsibilities mirror those of the MSW role, including psychosocial assessment, treatment planning, therapeutic interventions, discharge planning, crisis intervention, and resource referral. The position requires leadership to achieve patient goals and outcomes, guided by the NASW Code of Ethics and the Swedish Mission and Vision.
Required Qualifications- Master's Degree in Social Work from an accredited school
- Upon hire:
Washington Clinical Independent Social Worker License - 1 year of experience in acute medical or mental healthcare in an inpatient or outpatient setting
