Bilingual Field Based Community Health Worker; CHW - King County, WA

Role Overview: The Field Based Community Health Worker (CHW) acts as a liaison with Medicaid members, providing home and social assessments, member education, and support to access appropriate care. They primarily engage face-to-face or via phone, fostering positive relationships, encouraging healthcare participation, and connecting members with community resources. The role involves community outreach, managing multiple tasks, and understanding diverse community cultures.

Responsibilities include: Creating positive member experiences, promoting health self-management, supporting social connections, conducting outreach, and managing time effectively. Flexibility to work in community settings and some evenings/weekends is required.

Qualifications: High school diploma or GED, 1+ year in community resources, bilingual in Spanish and English, reliable transportation, and proficiency in MS Word and Outlook. Preferred candidates have community health experience, relevant certifications, or backgrounds in social work, nursing, or health advocacy. Candidates must reside near King County, WA, and be able to travel 50-75% of the time.
