Job Summary:

At Carease Health - IL, we prioritize proactive measures for enhanced physical health and well-being by incorporating Remote Patient Monitoring (RPM), Chronic Care Management (CCM) and Collaborative Care Management (CoCM) into our comprehensive preventative care program. As a Social Worker/Care Companion, you will play a crucial role in providing emotional support, brief therapeutic interventions, and mental health assessments to residents in skilled nursing facilities.

If you're looking for a meaningful, resident-focused work experience in a supportive team environment, we invite you to join our Collaborative Care team.

Key Responsibilities:
  • Visit designated facilities as scheduled by your supervisor to conduct assessments of residents' emotional, cognitive, and psychosocial needs.

  • Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns.

  • Document visit notes and maintain detailed records of interactions and interventions after each visit.

  • Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP.

  • Understand geriatric mental health, crisis management, and psychosocial support in long-term care settings.

Qualifications:
  • Associates, Bachelor's, or Master's degree in Social Work, Psychology, Counseling, or another mental health-related field.

  • Strong communication, documentation, and relationship-building skills.

  • Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings.

Salary:

$14,000 - $16,000 ANNUALLY
