• Provide direct patient care, including assessments, medications, and treatments
• Develop and implement individualized care plans in collaboration with the healthcare team

• Maintain accurate and timely documentation of patient records and treatments

Requirements:

• Minimum experience:

• License/Certification:
  Current LPN/RN licensure in the State of Illinois
