Continuum Therapy Partners is seeking a compassionate and dedicated Physical Therapist (PT) for a consistent PRN opportunity at Evergreen Nursing and Rehab in Effingham, Illinois.

This position offers a predictable schedule of approximately 2-3 days per week (8-12 hours per week) with the flexibility to consider part-time or full-time opportunities as well.

Join a supportive, collaborative skilled nursing facility where your clinical expertise makes a meaningful impact every day. As a Physical Therapist (PT), you will provide high-quality therapy services to help patients improve mobility, strength, balance, and functional independence.

• Evaluate patients and develop individualized physical therapy treatment plans.
• Provide skilled physical therapy interventions to improve mobility, strength, balance, and independence.
• Perform therapeutic exercises, gait training, mobility training, and functional activities.
• Monitor patient progress and adjust treatment plans based on patient needs.
• Collaborate with physicians, nursing staff, caregivers, and interdisciplinary rehabilitation team members.
• Complete accurate and timely documentation according to company, state, and federal guidelines.
• Educate patients and families on mobility strategies, exercise programs, and safety techniques.
• Provide compassionate, patient-centered physical therapy services.

• Graduate of an accredited Physical Therapy program.
• Current Illinois Physical Therapist (PT) license required.
• Skilled nursing or rehabilitation experience preferred but not required.
• New graduates are encouraged to apply.
• Strong communication, organization, and teamwork skills.
• Passion for providing high-quality patient care.
