Social Services Director

Job Summary

Effingham Healthcare & Senior Living is seeking a compassionate and experienced Social Services Director to join our team. As a Social Services Director at Effingham Healthcare & Senior Living, you will be responsible for providing high-quality, person-centered care to our residents, while also overseeing the social services department and ensuring seamless admissions and transition processes.

• Provide quality compassionate medical social services to our residents, ensuring compliance with all regulations and standards
• Coordinate admissions for new incoming residents, ensuring a smooth and welcoming experience for all new residents. Assists families with the move in process
• Coordinate with healthcare professionals, family members, and other stakeholders to ensure resident-centered care
• Provide emotional support and guidance to residents, family members, and staff
• Oversee transitions to home, working with home health providers and service providers to ensure services needed are setup prior to discharge
• Collaborate with other departments, such as nursing and activities, to ensure a comprehensive approach to care
