Full Time. Salary Range: $35.10 - $36.05 per hour. Job Shift: 1st shift.

• Conduct initial assessment of prospective residents, including off‑site visits.
• Perform wellness assessments and develop individualized care plans based on the needs and preferences of each resident.
• Deliver health promotion services, offer disease prevention counseling, and provide self‑care education to residents.
• Engage with residents and family members in a compassionate and impactful manner, creating a lasting and positive experience.
• Foster communication by regularly engaging with physicians, pertinent healthcare providers, and family members.
• Hire, train, schedule, and lead the clinical team in providing exceptional care experiences.
• Coordinate the Community’s infection control program while maintaining state regulatory compliance.
• Develop and oversee the Community’s medication management program.
• Contribute toward quality assurance and performance improvement initiatives.
• Comply with all rules and regulations regarding confidentiality and privacy of resident information.

• Maintain current state nursing licensure; LPN required, RN preferred.
• Minimum of two (2) years nursing supervisory experience.
• Prior senior/supportive living experience strongly preferred.
• Excellent written and verbal communication skills.
• Must possess leadership and supervisory ability and readiness to mentor, motivate, and inspire team members.
• Demonstrate exceptional interpersonal skills and ability to serve with love, compassion, and dignity.
