Transitions Hospice in Illinois is seeking an RN Case Manager to serve Effingham and surrounding areas. You will be the clinical anchor for patients and families, coordinating care with an interdisciplinary team and showing up when it matters most.
The role requires an active RN license in the state, nursing school graduate, and ideally 1+ year hospice/home health experience. You will conduct assessments, develop care plans, administer medications, document care, and educate families.
