Position Summary
The Nurse Care Manager utilizes expert clinical skills and a disease-specific knowledge base to assist identified patients through the continuum of care in an effort to provide appropriate, quality care in a cost‑effective manner. The Nurse Care Manager conducts daily assessments to identify high‑risk patients and manages their care during the acute care episode.

Qualifications- Education: Graduate of an accredited school of nursing required. Bachelor's in Nursing preferred.
- License/Certification: Current licensure as a Registered Nurse in the State of New Jersey and BLSHCP Certification required within 6 months from date of hire or transfer. For some specialties, a Master's degree in Nursing and an advanced practice nurse certification may be required.
- Experience: 5‑7 years clinical experience required. Prior case management/insurance management experience preferred. Excellent interpersonal, written communication skills, computer literacy, analytical and delegation skills required. Proficiency in Clinical Applications preferred at time of hire; incumbents within the position will be trained appropriately and then required to possess this skill within 30‑60 days from date of hire.
- Performance Expectations: Demonstrates the technical competencies as established on the Assessment and Evaluation Tool.
Potential for exposure to hazards and risks of the hospital environment, blood‑borne pathogens, including exposure to infectious disease, hazardous substances, and potential injury. This position requires frequent walking and standing.
Reporting RelationshipThe Nurse Case Manager at Mainland reports to the Case Management Divisional Manager, and the Nurse Case Manager at City reports to the Case Management Director.
